Nursing Home Non-Medical Transportation
This program is funded by the Long Term Care Ombudsman Program and administered by Senior Resources Agency on Aging. The Long Term Care Ombudsman Program has given Senior Resources money to administer the social transportation program during the upcoming year 2008. The Long Term Care Ombudsman Program began a pilot program several years ago aimed at improving residents’ access to social (non-medical) transportation. The Long Term Care Ombudsman Program, in conjunction with Senior Resources, designed this non-medical transportation program to assist nursing home residents in need of social transportation. This program has been quite successful over the years. This is a state wide program. Any resident in a skilled nursing facility in Connecticut qualifies to use this program. The nursing home applies for the “trip” on behalf of the resident. Requests must be submitted on the required application. When a resident wants to go on a social outing the following steps are to be taken by the nursing home staff before submitting the application:
