Multi-Disciplinary Teams (M-Teams)
Senior Resources - Agency on Aging provides four multidisciplinary peer networks (called "M-Teams") in Eastern Connecticut. M-Teams focus on elder issues. Membership is free and open to persons who serve the aging population, such as case managers, senior center personnel, care facility staff, health care providers, housing staff, discharge planners and the like. Members are expected to maintain regular attendance. Meetings allow an opportunity for confidential case discussion. The M-Team is an ideal opportunity to get help with a challenging case, learn about upcoming events and discover vital resources. It's also an ideal platform for members to promote their specific services and agencies. M-Team Meeting Schedule 2007 - 2008:Southeast M-Team meets in New London County on the first Tuesday of the month at 8:00 am. Meetings typically feature guest speakers and confidential case discussion.
Estuary M-Team meets at 9:00 am on the first Wednesday of alternate months at Saye Brooke Village, located at 55 Sheffield Street in Old Saybrook. Meetings typically feature guest speakers and confidential case discussion.
Windham M-Team meets on the first Thursday of the month at 8:30 am in Windham County. Meetings feature confidential case discussion. On alternate months, these meetings are followed by the Windham Area Elder Provider Network, which typically feature guest speakers.
Northeast M-Team meets on the first Friday of alternate months at 9:30 am at Creamery Brook, located at 36 Vina Lane, Brooklyn CT, (860)779-8700.
