Midstate Healthy Living Expo 2010 will offer health screenings. The Committee has decided there will
not be any invasive screenings done (those requiring blood) at the Expo. Following is a list of
possible screenings:

Have you considered becoming a Sponsor? We have new and exciting sponsorship opportunities available! (Please note: Sponsorship fee includes the table cost)

Sponsors are given prime locations at the expo. All other agencies requesting a different location will be denied.

If you wish for your organization to be a sponsor, please fill in the number "1" in the level of sponsorship of your choice.

Platinum Sponsor $800.00

• Given prime location at the Expo
• Your Agency logo (can be linked directly to your Agency) will be placed on Senior Resources website page for 6 months! Average yearly traffic of over 36,000 unique visitors
• Listed on a THANK YOU board which is prominently displayed on the day of the Expo
• Agency name is included in local newspapers, radio and cable advertising prior to the Expo
• Given an opportunity to participate in a live remote interview regarding their agency the day of the Expo

Gold Sponsor $600.00

• Agency name is included in local newspapers, radio and cable advertising prior to the Expo
• Your Agency brochure (no larger than 8 ½“X11”) will be placed in Senior Resources Information Center for 6 months! Foot traffic is visited by Consumers and Providers within our 56 town region
• Given an opportunity to participate in a live remote interview regarding their agency the day of the Expo

• Agency name is included in local newspapers, radio and cable advertising prior to the Expo
